Among other services, our site is fully-equipped and available to provide on-site lab services, x-rays, EKGs, spirometry tests, ultrasounds, venous doppler studies, holters, echos, stress tests and physical therapy. In addition, we offer on-site appointments for specialist’s consultations and follow-ups.

Urban Family Practice to Test Patients for Coronavirus
Buffalo, NY – April 2020 With Erie County confirming over 500 cases of Coronavirus, also known as COVID-19, there is an increased pressure within the community to enhance testing and treatment capacity. While these numbers are dwarfed by the active cases plaguing New York City and the downstate area, health officials in Erie County know the worst is yet to come. Urban Family Practice is doing everything they can to prepare for the uptick in cases in the coming weeks.
On April 1st, Urban Family Practice began conducting COVID19 tests on their high risk patients that have been monitored via telehealth by their physicians after experiencing symptoms or possible exposure to others living with the coronavirus. Risk factors include age (50+), chronic conditions such as Chronic Upper Respiratory Diseases (COPD) or Asthma, and current health status. Dr. Vazquez and his team arrived at the building early in the morning to set up a drive thru station based on CDC recommendations.
Security staff Angel Gonzalez, Angel Montalvo and Tyrone Bradley greeted patients at the parking lot entrance and relayed their information to the front line testing staff. Patients were then directed to drive under a tent, where they remained in their vehicles as office managers Terrance Gaiter and Debbie Roman performed a nasal swab. The testing, completed in less than 5 minutes, was then sent off to the lab for results. While patients await results, nurses and physicians continue to utilize the robust telehealth systems to monitor symptoms and medications.
“There is a lack of testing in the East and West Side of Buffalo. Our communities are under-tested and underrepresented,” said Dr. Vazquez, President and CEO of Urban Family Practice. “The numbers are low, but so are the number of tests that have been conducted. The only way to get ahead of this is to have actual numbers and know what we are dealing with.”
Upon the first case reaching Erie County, Dr. Vazquez and his team set up a nurse hotline to tackle influx of calls related to the coronavirus. Anyone experiencing flu-like symptoms including fever, cough, or shortness of breath is encouraged to call our nurse triage line at (716) 604-0504 to set up a telehealth video visit. From there, a physician will determine if testing is necessary, and if so, an appointment will be made at our drive-thru clinic.

BUFFALO, NEW YORK, July 30, 2019— Urban Family Practice is pleased to announce that it has been awarded NCQA’s coveted Patient-Centered Medical Home recognition. Medical practices that are bestowed this honor demonstrate that their patient-centered medical home is a model of care that puts patients at the forefront of care and builds better relationships between patients and their clinical care teams.
The NCQA’s Patient-Centered Medical Home Recognition program is the most widely adopted medical home evaluation program in the country.
