We accept most major insurance plans. All co-pays are due at the time of service.
Payment in full is expected at the time of service. We accept cash, check or MC/Visa. (There will be a $20.00 fee for any returned checks.)
If you are an HMO member, your insurance company has certain rules that you need to follow.
You must contact your PCP (Primary Care Physician) prior to going to the emergency room at any time of the day or night (unless it is life threatening).
You must see your PCP prior to any specialist to obtain a referral.
If you need any doctor’s notes, immunization records, prescription refills, physical forms, etc. kindly give our office 24 hours notice to prepare these for you.
