Dr. Raul Vazquez is a primary care physician who opened Urban Family Practice on Buffalo’s Lower West Side in 1996. In 2016, the National Committee for Quality Assurance (NCQA) announced that Urban Family Practice of Buffalo had received level three NCQA Patient-Centered Medical Home (PCMH). Level three is the highest degree of recognition available.

Recognizing the need for coordinated, person-centered care for Medicaid recipients with multiple chronic medical conditions, he formed the Greater Buffalo United Accountable Healthcare Network (GBUAHN). The New York State Department of Health approved the designation of GBUAHN as a provider-lead Medicaid health home in 2012.

As president of GBUAHN, Dr. Vazquez leads a team of dedicated professionals in the medical and business fields. He has set the strategy and direction for the fast-track growth of GBUAHN. Under his guidance, GBUAHN has grown to serve several thousand members and employs a staff of approximately 150 individuals. GBUAHN is one of the largest minority employers in Western New York.

In June 2009, Dr. Vazquez was nominated by Governor David Paterson, and approved by the New York State Senate, to serve as a member of the New York State Medicaid Advisory Committee. In June 2012, Governor Andrew Cuomo and the New York S Senate approved Dr. Vazquez to serve on the Minority Health Council.

He has won more than 20 awards for his accomplishments in business, the medical field and advocacy groups. Dr. Vazquez has served boards for numerous organizations, including: Upstate New York Transplant Services, HEALTHeLINK and WNED-TV.

Dr. Vazquez received his undergraduate degree from Fordham University and received his medical degree from the State University of New York at Buffalo.

Toni Vazquez is the chief of systems for GBUACO, co-owner of Urban Family Practice, co-founder of the Raul and Toni Vazquez Foundation, and the current president of the Williamsville Schools Board of Directors. Mrs. Vazquez is a philanthropist and inspirational leader for aspiring business professionals. She is the recipient of several prestigious awards, including: 2016 Business First Women of Influence award; the 2016 25 Influential Black Women in Business award from Black Western New York Magazine (BWNY Magazine) and (along with her husband, Dr. Raul Vazquez), the 2015 Buffalo Urban League Family Life Award.

Chief Medical Officer (GBUAHN)

Dr. Fox is chief medical officer (CMO) for the Greater Buffalo United Accountable Health Network (GBUAHN). Dr. Fox started as a solo rural family physician in a small village near Canandaigua, New York. After practicing there for 12 years, he went into academic medicine in 1993 as associate program director for the Deaconess Family Medicine program, run in conjunction with Buffalo General Hospital.

After successfully rebuilding the program from eight to 20 residents, and increasing the faculty complement from two full-time faculty to six full-time faculty, he began a career in primary care research. His area of interest is improving care at the doctor’s office level and translating evidence-based medicine into practice utilizing practice-based research networks. He is considered an expert in the field of practice-based research.

Dr. Fox is recognized in both the United States and Canada for his expertise in chronic kidney disease. He is a Fellow of the National Kidney Foundation (NKF) and became the first primary care physician ever to serve on the national board of the NKF.

Dr. Fox was a principal investigator of a $3.7 million National Institute of Health study using computer decision support to improve the early recognition and treatment of chronic kidney disease. In 2015, his research work earned him a university award in recognition of outstanding achievement in scholarly activity.

In 2001, Dr. Fox founded the Lighthouse, a free clinic run by medical students. He also founded the Forensic Medical Unit for the Family Justice Center, a one-stop shop for domestic violence victims. In 2003, Dr. Fox became a charter member of the Richard Sarkin Chapter of the Gold Humanism Society.

Dr. Kenneth Lee Gayles is a cardiovascular specialist who treats over 6,000 cardiac care patients annually. He is the owner of Gayles Medical practice. Dr. Gayles is board certified in cardiovascular diseases and internal medicine and brings to GBUAHN over four decades of experience diagnosing and treating congenital heart defects, heart failure, coronary artery disease, and other heart problems. He was the first African-American invasive cardiologist in Western New York.

As senior vice president of GBUAHN, Dr. Gayles’ mission is to deliver a comprehensive health care system targeting the underserved community in Western New York. His goal for GBUAHN is the elimination of disparity in health care delivery and outcomes in the Western New York community.

Dr. Gayles served as chairman of the board of Sheehan Memorial Hospital from 1990 -1991 and served as medical director until 1995. He earned his Bachelor of Science degree in biology and his medical degree from the State University of New York at Buffalo. He completed his internal medicine residency and cardiology fellowship at SUNY Buffalo.

Dr. Gayles has served as treasurer of the National Association of Black Cardiologists, and as vice president of the National Medical Association - Buffalo Chapter. His other community affiliations include the Buffalo Philharmonic Board of Directors and the Buffalo PREP Board of Directors. He is a trustee for the First Shiloh Baptist Church and has served as chief of medicine at Deaconess Hospital. He is also member of the Board of Trustees of the UB Foundation.

Kimbo B. Chia, M.D. is a practicing surgeon in Buffalo, N.Y. Dr. Chia graduated from University de Yaounde I in 1978 and has been in practice for 38 years. He currently practices at Urban Family Practice and is affiliated with Kaleida Health Buffalo General Hospital and Mercy Hospital of Buffalo. Dr. Chia accepts multiple insurance plans including Medicare, Medicaid and United Healthcare. Dr. Chia is board certified in surgery. In addition to English, Dr. Chia's practice supports Spanish.

Anita Joshi had been with Urban Family Practice as a Physical therapist for last 14 years. She graduated from M S University in India in 1983 in physical therapy. She got her New York State license to practice in 1985. She has had 33 years of experience in field of physical therapy. She had worked at Sisters of Charity Hospital Buffalo Nee York for 4 years. In 1990 she started her own private practice. She worked as a senior therapist in several area nursing homes caring for geriatric clients. At present she specializes in treating orthopedic and neurological clients. She continues to accepts new patients. I am very proud to be a part of team at Urban Family Practice.

Dr. Jesslyn Perry has been a physician since 1995. After working at Sheehan Hospital as an internist from 1999 -2001, she opened Ella Perry, M.D., located at 191 North Street in Buffalo, N.Y. She named her practice after her mother.

Dr. Perry specializes in family and internal medicine. She is affiliated with Women and Children’s Hospital of Buffalo and Buffalo General Hospital. She is board certified in Internal medicine. Dr. Perry is a member of the National Medical Association (NMA).

Dr. Perry is a graduate of the State University of New York at Buffalo School of Medicine and Biomedical Sciences.

Daniel Fells graduated from the D’Youville College Physician Assistant Program in May 2004 and has been working with Dr. Raul Vazquez at Urban Family Practice for the last thirteen years.




Casey Feeney, PACasey Feeney, PA
Physician Assistant

Casey Feeney is a board certified physician’s assistant who was born and raised in Buffalo, N.Y. She graduated from D’Youville College in 2017 with a Master’s of Science in physician assistant studies. She also earned a Bachelor of Arts in psychology from D’Youville College in 2009; and graduated from University at Buffalo with a Master’s of Arts in behavioral neuroscience in 2012.

Feeney began rotating at Urban Family Practice as a student under the supervision of Dr. Raul Vazquez and Daniel Fells, PA-C. During that time, she developed a great appreciation and love for the practice of family medicine in an urban setting.

Feeney began practicing at Urban Family Practice as a provider in February 2018. She hopes to continue to grow as a provider at Urban Family Practice.

Brendan Farnand, PABrendan Farnand, PA

Brendan Farnand grew up in Rochester and moved to Buffalo to attend university in 2013. He received his BS/MS in physician assistant studies in 2017 from D’Youville College. Before graduation, he completed three clinical rotations with Daniel Fells at Urban Family Practice starting in early 2017. He also speaks fluent Spanish, after living for a time in Colombia.

Latasha is currently Board Certified, and earned her Master’s degree in Science as a family nurse practitioner from D’Youville College in 2017. She also has 12 years’ experience as a registered nurse (RN) with extensive experience in vascular special procedures and med-surg nursing. Prior to nursing she also spent three years as a medical assistant in a critical care unit.

Sarah is married and is a mother to a feline and k-9. She thoroughly enjoys traveling, hiking, kayaking, and everything outdoors. She grew up in Pennsylvania and is now enjoying exploring the Buffalo area. Sarah went to Clarion University in Pennsylvania, receiving a bachelor’s in nursing, and graduated from Missouri State University in 2017 with a doctorate in nursing practice. She deeply cares about the people she helps and loves to be clinically involved with them.


Isharon Reynolds, FNP-CIsharon Reynolds, FNP-C

Isharon Reynolds is a family nurse practitioner who specializes in family medicine. Isharon is currently completing a fellowship through Midlevel U at Urban Family Practice. She completed her Bachelor of Science in nursing at Kent State University. Following graduation, Isharon worked in multiple intensive care units in the Greater Cleveland area including the Cardiovascular Institute at Clinic Main Campus, Surgical ICU at University Hospitals and the Trauma ICU at MetroHealth Medical Center Level 1 Trauma Center. Isharon earned her Master of Science degree at Ursuline College in Pepper Pike, Ohio. Isharon holds a national board certifications with the American Academy of Nurse Practitioners. (AANP) as a family nurse practitioner.

Dr. Nicholas A. Howard is an accomplished pediatrician, holding medical doctorates from the American University of Antigua and Xavier University in Aruba. He is American Red Cross certified for advance cardiac life support and pediatric advance life support, as well for neonatal resuscitation.

In 2012, Dr. Howard became a research assistant at the Women’s and Children’s Hospital of Buffalo for their Prematurity and Respiratory Outcomes Program (PROP). The goal of the study was to identify bio-chemical, physiological and genetic biomarkers as well as clinical variables associated with pulmonary status in preterm infants. Dr. Howard continued as a pediatric resident, caring for pediatric patients on the inpatient floors, emergency room and outpatient clinics. He also performed multiple procedures including intubation, lumbar punctures, placing on both central and peripheral lines. Additionally he was responsible for the education of medical students on the inpatient floors and clinics.

Now, at Urban Family Practice, Dr. Howard oversees the provision of the most up-to-date medical treatment and education of the pediatric population, effectively addressing the psychosocial, cultural and ethical concerns of our patients.

Our surgeons specialize in minimally invasive and robotic surgery as well as the traditional open method. Dr. Adams, Dr. Barone and Dr. Falvo all completed their general surgery residency training at SUNY Buffalo, followed by Colorectal Surgery Fellowship, before coming back to Buffalo and joining Delaware Surgical Group.
